EMPLOYMENT OPPORTUNITY: Community Connector for Older Adults

Through funding made possible by Saskatchewan Seniors Mechanism, the Town of Shaunavon has entered into an agreement to be a pilot initiative for Social Prescribing for Older Adults. This 3.5 year initiative will include the hiring of a half-time (20 hours/week) contract position to implement social prescribing for older adults in the community.

Job Summary: Social prescribing is a way for medical professionals to connect individuals to a range of non-clinical services in the community to help improve the health and well-being of older adults. It addresses underlying non-medical causes as it promotes integrated community-based support and care and helps to reduce some of the dependence on health service providers. A Community Connector will empower older adults to take control of their health and wellbeing through a holistic approach by connecting this population with community groups and services for practical and emotional support.
